PURPOSE

The purpose of Requirement 201 is to compile (or to update a previous preliminary hazard list (PHL)) a list of potential hazards very early in the system development cycle on which management emphasis needs to be placed.


REQUIREMENT

Examine the system shortly after the concept definition effort begins and compile a list (PHL) identifying possible hazards that may be inherent in the concept and their associated mishap potential, or identify hazards specified by the MA.


Review safety experience on similar systems, including mishap/incident hazard tracking logs (if accessible), safety lessons learned, etc., to identify possible hazards and their mishap risks.  The sources of a hazard found in this review shall be referenced in the PHL.


Further investigate selected hazards or hazardous characteristics identified in the PHL as directed by the MA to determine their significance.


REPORT REQUIREMENTS

The contractor shall prepare a Report that contains the results from the work task described by paragraph "REQUIREMENTS" above to include the following information:


Hazard Analysis Results

 This will consist of a summary or a total listing of the results of hazard analysis.


Contents and formats will be as agreed upon between the contractor and the managing activity(MA).


The following are the content requirements unless otherwise modified: 


A summary of the results.


A listing of identified potential hazards, in narrative or matrix (sometimes called columnar or tabular) format, to include the following information


Hazard Description

A brief description of the hazard or hazardous material; for example, "Radiation leakage from radar set waveguide."  


The recommended action required to eliminate or control the hazard.


Any information relating to the hazard not covered in other blocks; for example, applicable documents, previous failure data on similar systems, or administrative directions


DETAILS TO BE SPECIFIED

Details to be specified in the SOW shall include the following, as applicable


Imposition of Requirements 101 and 201.


Identification of special concerns, hazards, or undesired events the MA wants listed or investigated.
